Be an Angel for Alzheimer’s

· In-home respite care (non-medical services) for caregivers will be provided when funding is available.

Eligibility Requirements:

· Diagnosis Alzheimer’s dementia

· Low income, per HUD income limits

· Resident of Glynn or surrounding counties—McIntosh, Camden, Brantley

· Home evaluation will be done to determine needs.

· A plan for in-home assistance will be developed according to individual family needs.

· Services may include patient-sitting, meal preparation, grocery shopping and errands, transportation, light housekeeping, medication reminders.

· A licensed, bonded, insured respite service will provide services.  Training in dementia care is required of respite-care providers.

· Alzheimer’s of Glynn/Brunswick will be billed for services, and will pay for up to 36 hours of in-home care per family.

 

Respite Program
